New Case Submission Form in the Portal

The case submission form has been rebuilt around the restoration type you select, so you only see the fields that apply.

Crown and bridge, implant, and removable cases each get their own field set. Tooth selection moved to an interactive chart, and shade fields now appear inline rather than in a separate section.

Your saved preferences carry over. The old form remains available from the case list for one more month.
